one of our many service locations, make sure that you tell them that you are with the

You will need
to contact our member service number to set up your first visit with one of our medical
providers. Ask for an AmeriPlan provider to receive
the largest savings.
All of your
lab work will need to go through lab corp to receive the largest savings. Make sure that you ask your doctor to use lab corp for all
labs that are sent out of office. Medstat will be
handling all of your lab services. Their toll free
number is 800-808-6511
If you have
to have other ancillary services (such as c-scans or MRI) done you will want to contact Ameriplan member service to find
the closest location to you.
